fix install-guide gnocchi+keystone instruction
...
gnocchi+keystone should be setting keystone configurations under
keystone_authtoken for api authentication. service_credentials is
for accessing external services

chill out on the number of items in toc
...
we have an absurd amount of entries in our table of contents[1]. this
patch does better groupings so we nest a bit more.
