Remove keystoneclient exception usage in tests

This change replaces the usage of keystoneclient exceptions in the
auth_token_middleware unit tests to use the ConnectFailure exception
from keystoneauth.

This is part of the process of removing keystoneclient from
keystonemiddleware.
