Stephen Finucane 25545d7f55 Remove 'bdm_(update_or_create|destroy)_at_top' ... These were being called within the 'BlockDeviceMapping' object. Remove them in their entirety, given that nothing should be calling them now. Note that this effectively breaks cells v1 but that shouldn't matter, given that it's no longer possible to use the thing.
