Update unique constraint of hosts table
In 'hosts' table hostname should be unique irrespective of failover_segments. So updated the unique constraint from "failover_segment, name, deleted" to "name, deleted". Closes-Bug: #1614081 Change-Id: I6b11a8e4bb5751c9ff5ec600185222ba55b67a2a
